Medical reports may be requested from the clients GP.

A PMAR (Personal Medical Attendance Record) and BMR (Brief Medical Report) are requests for medical information from the clients doctor with the clients signed consent. Health Predictions case managers regularly follow up with the clinics to ensure the reports are available in a timely manner thereby supporting the successful implementation of the clients insurance application.

Commonly requested reports:

- PMAR and BMR

- Results of past medical or pathology testing

- Medical record file copy

- Questionnaires

PMAR 

A Personal Medical Attendant Report (PMAR) is a full copy of the clients medical file. Their GP is requested to provide historical medical data from their own files which may include reports from specialists who they have previously referred their patient, our client, to for specialist advice and care. 

BMR

A Brief Medical Report (BMR) is requested when specific information on a past illness or injury is required. Your clients GP can usually provide sufficient history for this report either from their own files, or from reports received and on file from specialist medical practitioners. 

File Copy 

A file copy refers to a clients full medical record held by the clients GP or medical clinic. Medical records include comprehensive notes relating to client consultations and other encounters with the practice.

Questionnaires

Specific questionnaires may be requested from the treating practitioner on topics such as mental health, joint and musculoskeletal impairment and diabetes, in order to obtain more detailed information which may be relevant to their current health.
